Independent Facilitator Services
(in-person or hybrid)
Individuals enrolled in the Self-Determination Program with Alta California Regional Center receive support and guidance creating person-centered service plans and navigating the Self-Determination Program, which include helping identify clients' needs, coordinating services, developing budgets and promoting goals. Supported Living Services
(in-person only)
Clients receive assistance with activities of daily living including personal health & hygiene care, mobility support, meal prep & feeding, fitness & exercise, light housekeeping & lawn care, pet care, as well as personal transportation for errands and appointments. Clients are also provided with basic companionship and advocacy support.
Education & Employment Coaching
(in-person or online/hybrid)
Clients receive support and guidance creating life-action plans through active-listening, guided self-reflection, person-centered planning and help discovering and exploring interests, options and resources that promote their education and/or employment goals.
